Resources to support the implementation of the guideline on older people with social care needs and multiple long-term conditions
The National Institute for Health and Care Excellence (NICE) have announced the publication of a suite of products to support the implementation of the NICE guideline on older people with social care needs and multiple long-term conditions.
The products published include:
- a tailored resource focusing on the named care coordinator role from the perspective of older people, and some associated practice examples
- a .PDF report of a round table discussion to help you find out more about the guideline and to give you ideas about how to implement it
