Our NEW service partner, Home Spirit, has launched a newly developed portal designed to improve outcomes for people living with dementia. The portal is a place where you can create goals, record ideas and make plans. The online integrated set of tools aids in understanding of dementia and assists in providing the best possible care and support for individuals.
The Home Spirit Dementia Audit tool is perfect for support at home providers in Hertfordshire. It has a Fully Funded trial access and we think it provides really good value for HCPA members at a cost of between £19 to £45 per month. We have recently teamed up with Home Spirit as a partner service of the association to offer you this tool which will enable you to:
- Reduce hospital admission and risk of falls through environment assessment
- Support your clients to function optimally in their home environment through environment assessment
- Support your clients to build networks of support
- Support your staff to observe better, and report better through bite size learning
- Access all of our infographics as and when they are published for training and learning
