The Care Home Impartial Assessor has now been working in Lister hospital for 2 months and we have a new assessor due to start in November which means this service will become a six-day service, Monday to Saturday. This role (previously known as ‘Trusted Assessor’) has been changed to emphasise that the Assessors are employed primarily to work impartially on behalf of residential homes.

Building trust, saving time and money
Here is an example of how the assessor role is benefiting providers;
A service user was identified to Heather (Impartial Assessor). Currently, the patient was not medically fit for discharge. However, to maximise relationship building, Heather made direct contact with the care home a week before the service user was discharged.
Heather introduced herself to the Deputy Manager and explained the Impartial Assessor role and how it has the potential to ease pressure on the care homes.
The Deputy Manager agreed for Heather to conduct an impartial assessment but was adamant that they would also need to conduct their own. The rationale of them wanting to conduct their own assessment was a result of past experiences of poor discharges from the hospital and therefore having to frequently re-admit their service users.
The comprehensive assessment was conducted the following week. The assessment was submitted directly to the care home’s Deputy Manager.
Heather contacted the Deputy Manager to gather feedback from the assessment. The Deputy Manager stated that the information provided was more than sufficient and gave great insight into her development and original baseline of their service user. The care home has stated that they would like all assessments to be done by the Care Home Impartial Assessors in the future.
Through key engagement and a thorough impartial assessment, the service user was able to be discharged the same day allowing a safe and effective discharge.
