In partnership with Hertfordshire County Council (HCC) and the Clinical Commissioning groups (CCG), HCPA are working on a number of projects to improve care for people with complex care needs.

The Kings Fund have released a review looking at the current picture for elderly social care following the government cuts and pressure within the NHS.

One of the interviewees from the review suggested that care workers were often being asked to carry out clinical tasks, or tasks which may have traditionally been carried out by a community nurse, e.g. stoma care or dressings. This suggestion rings true with the developments happening in Hertfordshire.

Through the Complex Care Framework it has been recognised that this is an issue for residential and nursing home care practitioners. Within these homes we have residents with higher clinical complexities that need additional care to improve or prevent progression of their conditions.

Care Practitioners are more than cable to complete some of the more clinical tasks, so the framework was developed to up-skill staff in the following areas;

  • Wound management
  • Falls
  • Respiratory conditions
  • Neurological conditions
  • Continence and stoma care
  • End of life care
  • Nutrition
  • Dementia

This successful project has;

  • Improved staff confidence to carry out new clinical tasks
  • Improved relationships with clinical professionals including GP’s and district nurses
  • Focused on prevention
  • Reduced admissions to hospital and emergency clinical care needed

All resulting in better care and outcomes for the residents.
