For everyone living in Hertfordshire with a Learning Disability

People with a learning disability often have poorer physical and mental health than other people, but this does not need to be the case.

Annual health checks are for adults and young people aged 14 or over with a learning disability. An annual health check helps people with learning disabilities stay well by talking about their health and finding any problems early so they get the right care.

People with learning disabilities do not have to be ill to have a health check. In fact, most people have their annual health check when they’re feeling well.

If people with learning disabilities are worried about seeing a doctor, or if there is anything health practitioners can do to make their visit better, let the doctor or nurse know. They’ll help make sure it goes well.

We want to find out what people with learning disabilities think about their Annual Health Checks. Please help us distribute and fill in the attached questionnaire about Annual Health Checks.
