Purpose: The goal of this survey is to collect feedback from managers and staff of provider services about their understanding of Connected Lives. Your responses will be collected and analysed to support the ongoing work of the Re-energise Connected Lives project by Hertfordshire County Council.
Our Connected Lives model puts choice, independence, enablement, community and citizenship at the heart of everything we do. Fundamentally it is a preventative approach that allows us to look at real and innovative solutions that enable people to live their lives to the full.
