In line with the Hertfordshire County Council’s ConnectedLives ethos, this approach includes a focus on implementing the necessary strategies to prevent any potential deterioration, and on supporting people, wherever possible, to improve (mentally, physically and socially), to gain independence, and to meet their desired goals.  

Providers will be supported to embed an ‘Enabling Care Culture’, and, through education and training, staff will learn to maximise the potential of the people in their care to be independent, using a strengths-based approach. All education is designed to facilitate the move from a ‘task based’ culture of care to an ‘outcome focused’ one.  

HCPA use Physiotherapists and Occupational Therapists to upskill your staff with enhanced knowledge and practical skills, which allow them to support individuals to improve such things as: sit to stand, mobility, and positioning, as well as how to spot early signs of deterioration.  

Staff will also learn how to maximise each person’s potential to engage in meaningful activities which enhance their wellbeing and quality of life.  

As well as signposting you to relevant fully funded training courses, this website also shares the most effective assessment tools and outcome measures to help reduce the risk of falls, assess for frailty, manage a person after they have fallen, and to monitor levels of activity and engagement.
