An assessment with multiple components that aims to identify a person’s risk factors for falling. This should replace all falls screening tools, which often miss people at risk of falls. On the MFRA, the more risk factors a person has, the higher their risk of falling. The MFRA should be part of an individualised, multifactorial intervention and should not involve a blanket approach for those risk of falls (World Falls Guidelines 2022, NICE, 2017): 

HCPA have developed a comprehensive MFRA tool for all Hertfordshire providers to use in place of their existing tools and this will be promoted through all relevant HCPA education. This may need to be cross-referenced with your digital care planning system falls assessment tools, as otherwise many of these forms may miss some of the risk factors for a person, resulting in inadequate strategies being implemented to mitigate a fall for that person.
