Advance Care Planning in End of Life Care – Residential & Nursing

We are delighted to host on behalf of Hertfordshire Hospices this interactive workshop on how to get the conversation started regarding Advance Care Planning. This course will be facilitated and be interactive looking at tools, templates for Advance Care Planning.

Learning Outcomes:
• At the end of the day, participants will be able to describe and discuss relevance of ACP to their own practice.
• Participants will have a knowledge of Key advance planning principles and tools.
• The participants will be able to recognize and use Key documents to support Advance Care Planning, and decision making.
• At the end of the day participants will be able to facilitate discussions with their service users/Residents and have an awareness of how to record this information. of ACP.
