As a person’s condition changes it is natural for them to experience some symptoms relating to their condition. There may be times however when a person with a learning disability is unable to tell us when they are in pain or feeling unwell. It may be useful to use a chart to enable those caring for the person to clearly document and be aware of non-verbal behaviours or signs indicating pain or discomfort. By noting these on the chart it will help care staff ensure they are comfortable and symptoms relieved either in hospital or in the community. This information should be kept close to the person for all members of the team to use. 

In this section there are a few of the more common symptoms present during end of life, although these will be dependent upon an individual’s condition and prognosis. It is important to document and monitor any symptoms experienced and discuss with the healthcare professionals involved how they can be minimised and managed.
