A session for Care Managers exploring how HCPA resources can support you to ensure Falls Risk Assessments are robust.

You will look at evidence-based falls and frailty interventions, and you will also discuss the importance of early frailty identification and assessment within your care setting.

You will also consider the use of Outcome Measures to monitor individuals’ progress or deterioration.

By the end of this session, participants will be able to:

  • Confidently use HCPA resources within their organisation to support more robust Falls Risk Assessments.
  • Identify the risk of frailty using evidence-based assessment tools.
  • Understand the purpose of Outcome Measures and implement these within their service.
