We are delighted to host on behalf of Hertfordshire Hospices a brief overview of future planning of care for service users with a life-limiting condition, frailty, a learning disability or multiple co-morbidities

Learning Outcomes:
• Develop an understanding of what Advance care planning is.
• Recognise the relevance, benefits and challenges of Advance Care Planning with relevance to delegate area of practice.
• Be aware of the key principles and some of the tools that can be used for effective Advance Care Planning ( Including ReSPECT
