The HCPA Prevention and Enablement Service aims to support all Adult Care Services across Hertfordshire to reduce the risk of falls and unnecessary hospital admissions, and to improve the independence and health outcomes of the people receiving care.
If you are an Adult Care Provider or an organisation that supports a Provider, you can use this Expression of Interest form to access our Targeted Support Service, which will provide your organisation with advice and guidance using our Prevention and Enablement Framework, as well as targeted education, along with to help you meet the standards required by CQC and HCC.
Targeted education topics may include, amongst others:

- Enabling Care
- Bed Prevention and Supportive Equipment
- Chair-Based Exercise
- Strength and Balance Exercise
- Engagement and Well-being
- Risk Assessments
