HCPA have gathered several local contacts and resources with the hospices so that any provider has the correct support at the right time.

The Palliative Care Referral Centre provides advice to healthcare professionals and clinicians (including care home staff), so should be considered as a first point of contact for palliative and end of life patients. They also provide a single point of referral and triage for all Palliative and End of Life referrals  and access via a central telephone number and email address

The service will operates:

Monday to Friday 9am – 5pm

Saturday, Sunday and Bank Holidays 10am – 2pm

There is also Specialist Palliative Care advice is available 24 hours a day via the Mount Vernon Specialist Palliative Care Advice line on 020 3826 2377.
