Advance Care Planning is often misunderstood, leading to confusion about responsibilities, documentation and what good practice looks like in care settings.

This funded information webinar will explore the myths and realities of Advance Care Planning (ACP), helping care providers understand how ACP supports people to express what matters most to them, both now and in the future. We will discuss why ACP is not just about end of life care, the difference between ACP and specific documents such as ReSPECT forms and DNACPR decisions, and how providers can play an important role in supporting meaningful conversations.

Through practical examples and myth-busting discussions, attendees will gain greater confidence in understanding the different elements of Advance Care Planning and how these work together to ensure a person’s wishes, preferences and choices are recognised and respected.
