In Hertfordshire this is ‘My Purple Folder’ but in West Essex there is a different version. My Purple Folder is a vital tool to support staff with specific person-centred information to help support an individual through any health appointment.
The Purple Folder should be used as a reference for a baseline for how an individual is when well. Although in relation to end of life care, it provides information of what is important to the individual overall in their life and what is important for them to be supported with in health appointments. It would be useful to review a passport in line with any end-of-life care plan which has been created.
