This Framework has been set up to provide social care providers in Hertfordshire the chance to upskill staff teams to enable them to support service users with more complex needs. The topic areas covered within this framework reflect the needs of the sector and aim to equip social care providers with the knowledge to;

  • Focus on ‘prevention’
  • Maintain abilities
  • Manage complexities
  • Understand how when and why to refer on (appropriately)

Staff undergoing the programme are reporting huge movements in confidence and morale and feel more equipped to cope with situations that arise with service users.

This Framework has been widened out to cover all types of services and to support providers across the spectrum of inspection status from inadequate right through to outstanding and its aspiration is to move services up the ladder by providing targeted support at the right time for the provider. Evaluation done at an early stage has shown that it is useful for providers of all sizes and also allows a provider to choose at what stage they will enter the Framework depending on their current capacity.

Choose from Foundation or Access options

Complex Care Foundation – starting September (6 Champion Pathways)

This stage of the programme is designed to enable homes who are wanting to improve their inspection rating and step into the ‘Premium’ stage of the Framework. You will be asked to identify a team of 6 people who are keen to learn and willing/able to pass information onto the wider staff team. The manager of the home will be engaged at the strategy days to enable them to support each champion pathway. The remit following the programme will be to cascade training onto all other staff in the home via continual short ‘Work Time Learning Sessions’ in all 6 areas.

Outcomes from the training the home can use to show on inspections…

  • CCF Team Meeting & Case Notes Weekly/Monthly showing differences made to service users
  • A developing  senior team
  • Lower Hospital Admissions to report
  • Lower Ambulance Call outs to report
  • Improved Service User Experience, personalised, focussed on wellbeing
  • Improved Family Communication/Involvement
  • Improved communication with clinical professionals and better management of health issues
  • Fewer behaviours that may challenge – less reliant on crisis teams/District Nurses and less anti-psychotic medication
  • Better Weight Management & Hydration & Reduction in Supplement
  • Fewer Falls, improved stamina, increased flexibility, improved muscle strength for Service Users – resulting from exercise programme

Complex Care Access – starting September (2 Champion Pathways)

This stage of the programme has been devised to support HpFT and ENHerts CCG in supporting homes who are identified as being ‘less able’ to manage clients living with progressing dementia. The home will be recommended to attend this programme by HpFT and ENHerts CCG and will be asked to identify a team of 3 people who are passionate about good dementia support in the home – this group will be made up of 2 senior care staff plus a member of activities staff. The manager of the home will be engaged at the beginning and end of the programme via the ‘culture change and scene setting’ training and on-site mentoring – ‘working with the CCA Team’. The remit following the programme will be to cascade training onto all other staff in the home via continual short ‘Work Time Learning Sessions’.

  • Complex Care Access Team Meeting & Case Notes showing the differences made to service users around dementia
  • A Developing  Dementia  team
  • Improved Service User Experience, personalised, focussed on wellbeing
