A message for learning disability services

Dear learning disability service provider,

Please find below a message from Louise Jenkins, Strategic Liaison Nurse for Secondary and Tertiary Care, regarding the new Hospital Prevention Project.

I am emailing to tell you about a new project that I am leading on from some short term STP money aimed at reducing winter pressures on the hospitals.

The objective of the project is to reduce hospital admissions for conditions that can be treated in the community by increased input by health professionals. So we have employed 3 nurses – Monica Charunda, Nicola Arthur and Anthony Chiari who have all had experience of community nursing (general and learning disability) who will be visiting providers to assess the individuals risk of hospital admission and develop Hospital Prevention Plans jointly with you as providers.

We hope to employ 2 more nurses to be based at Apsley but the project is countywide.
