DESCRIPTION:Course fee: HCPA Standard Member: Fully funded*\nAssociate/Network Member: £25+VAT | Non-member: £50+VAT \nHalf day course at HCPA Mundells Campus!\n \nA session for Care Managers exploring how HCPA resources can support you to ensure Falls Risk Assessments are robust. \nYou will look at evidence-based falls and frailty interventions\, and you will also discuss the importance of early frailty identification and assessment within your care setting. \nYou will also consider the use of Outcome Measures to monitor individuals’ progress or deterioration. \nBy the end of this session\, participants will be able to: \n\nConfidently use HCPA resources within their organisation to support more robust Falls Risk Assessments.\nIdentify the risk of frailty using evidence-based assessment tools.\nUnderstand the purpose of Outcome Measures and implement these within their service.\n\n*Please note: As per our T&C’s\, this course carries a fee in the event of non-attendance\, non-completion or late cancellation.\nDelegates who are late may not be admitted into the training and a cost incurred
