SUMMARY:New Homecare Falls Pathway Webinar
DESCRIPTION:2 hour webinar via Virtual Classroom\n\nNew Homecare Falls Pathway Webinar \nWhen a person has fallen:\n• Do you/your staff know when to call 999\, when to contact Avoidance of Admission Services\, or when it is safe to assist the person up from the floor?\n• Do you/your staff know how to safely support a person to get up after a fall? \nIt is extremely important that Emergency Services (999) are only contacted when necessary. Calling 999 inappropriately can place additional pressure on an already overburdened service\, potentially delaying response times for life-threatening emergencies. It is also important to recognise that not everyone requires – or wishes – to be admitted to hospital following a fall. \nThis webinar has been developed to support homecare providers in understanding local guidance and best practice for managing falls in a person’s own home. It will provide practical advice on responding to falls\, reducing associated risks\, and supporting individuals in a safe and person-centred way. \nThe session will introduce the Hertfordshire Management of a Person who has Fallen in Homecare Pathway\, a new local pathway designed to guide decision-making following a fall. It will also highlight the wide range of services\, resources and support now available to providers\, including how and when to access Avoidance of Admission Services in your area. \nSign up to build confidence in:\n• Making appropriate decisions following a fall\n• Using local services effectively\n• Safely supporting individuals to get up from the floor\n• Embedding best practice aligned to local guidance within your service \nLearning Outcomes:\n• Understand the key principles of the Hertfordshire Management of a Person who has Fallen in Home Care Pathway\n• Identify when it is appropriate to call 999\, access Avoidance of Admission Services\, or safely manage a fall within the home environment\n• Describe best practice approaches to supporting a person following a fall\, in line with local guidance\n• Recognise the importance of reducing unnecessary hospital admissions and supporting person-centred decision making\n• Navigate and access the range of local services\, resources and support available to homecare providers following a fall \nCLICK HERE TO REGISTER\n

The afternoon will cover crisis\, recovery\, and wellness plans. \nPlease note\, the substance misuse course has been co-designed with a professional from Change Grow Live Drug and Alcohol Service to raise awareness and consider how professionals can work and communicate with people with a mental illness and who misuse substances. It is not designed to train professionals to become drug and alcohol workers as these roles are highly specialised. \nIt will help learners to understand some of the many reasons why people misuse both legal and illegal substances. Learners will have an opportunity to learn briefly about a wide range of substances however\, focus will be placed on those substances most prevalent to Hertfordshire. \nLearners will have opportunities to consider some of the barriers people face regardless of a formal diagnosis and how people can be supported through these barriers. \nSpotting the signs of substance misuse is not always easy as some signs can often mimic signs of mental disorders\, therefore the course will outline a wide range of signs and symptoms. \nThe course will offer advice on how to share your concerns with a person they may know or suspect of misusing substances and how to work collaboratively with the individual. \nFurther advice will be given to learners on practical support they can provide including harm-reduction methods\, providing good information and advice. \nIn the afternoon learners will have the opportunity to discuss as part of a wider group what is meant by the term crisis and the impact a crisis can have upon a person’s mental\, emotional and physical needs\, including the impact on upon “External structures” E.G. family\, work\, finances etc. \nLearners will then explore what is meant by the term recovery and will consider two models: personal recovery and clinical recovery. \nLearners will consider the difference between a lapse in a person’s recovery and a relapse\, and how individual perspectives on either can cause confusion or distress. Learners will then explore signs that could emerge across a range of domains\, E.G. mental\, emotional etc. \nLearners will understand that wellness plans are not a plan for them to complete\, they are the responsibility of the person who has mental health difficulties/in recovery. \nLearners will look at the 5 principles of wellness plans and consider how\, through individual support\, group support etc\, individuals can be helped\, where needed\, to capture important information about their own crisis\, their own recovery and what helps and hinders their continued journey. \nLearning objectives – Substance misuse:\n• Understand what is meant by the term substance misuse\, types of legal and illegal substances people misuse including how to recognise them by their real or street name\, paraphernalia associated with their use and the immediate and long term affects of their use.  \n• Raise awareness of Naloxone\, an anti-overdose drug for opioids\, types and an introduction to how its administered for educational purposes only\, how to spot the signs of an opioid overdose and what to do.   \n• Consider the barriers to support people may face including stigma\, personal and healthcare systems and what we can do to help an individual to start to overcome these barriers.  \n• Spotting some of the signs and symptoms of drug abuse and learners will have an opportunity to identify and share through their own knowledge and experience\, other potential signs.  \n• How to constructively share your concerns with a person who you either know or suspect of misusing substances.\n• Consider several tips of how to work collaboratively with an individual.  \n• Consider statutory and non-statutory services and their referral pathways.  \n• 1-1 interventions \n• Recovery \n• Care and support planning.  \nLearning objectives – Crisis Recovery and Wellness Plans:\n• To understand what is meant by the term “Crisis”. \n• To consider how a mental health crisis may look and feel. \n• Consider what is meant by the term recovery. \n• Consider the differences between personal and clinical recovery and how these interact with one another.  \n• Look at the importance of self-management and support networks in recovery.  \n• Discuss the differences between lapse and relapse and what this can mean for recovery.  \n• A step-by step guide to how wellness plans are completed. \n• Group reflection on how wellness plans can be introduced to your organisation. \n \n*Please note: As per our T&C’s\, this course carries a fee in the event of non-attendance\, non-completion or late cancellation.\nBookings of more than two delegates per site may be amended to allow access to all member organisations at the discretion of HCPA.\nDelegates who are late may not be admitted into the training and a cost incurred

Champions mentor peers\, influence care delivery\, and maintain elevated care standards through ongoing governance and learning. \nWe suggest care providers have a Champion in each relevant topic area. To ensure quality and focus\, each individual may only be a Champion in up to two topic areas: \nThe Dementia CHAMPION course will enable Care professionals to take on the role of Champion within their organisation for this topic. The Champion will have a clear understanding of local and national guidance and legislation for and will cascade this information amongst care teams\, improving the safety and quality of care for individuals. \nLearning Outcomes:\n• Define the expectations and responsibilities of a Dementia Champion within a care setting\, and explain how this role can enhance service quality and culture.\n• Describe the principles of personalised\, outcomes-focused care planning in dementia care and explain their importance in meeting individual needs.\n• Define the principles of Positive Behaviour Support (PBS) and explain their application in promoting dignity\, autonomy and well-being for people living with dementia.\n• Select and apply evidence-based pharmacological and non-pharmacological interventions to meet the changing needs of individuals with dementia\, including the management of behavioural and psychological symptoms (BPSD).\n• Implement dementia-friendly environmental adaptations\, communication strategies and active support approaches that enable daily living\, foster positive engagement and reduce distress.\n• Demonstrate effective multi-disciplinary working and collaboration with families\, carers and professionals to support the person with dementia holistically.\n• Analyse case studies to identify challenges\, risk factors and opportunities for improving care\, and apply risk-positive approaches that encourage meaningful engagement.\n• Evaluate the impact of language on perceptions of dementia and analyse how communication choices influence stigma\, inclusion and person-centred relationships.\n• Assess methods for evaluating dementia care standards and measuring service impact\, reflect on insights from experts with lived experience.\n• Develop SMART goals and a practical action plan to drive positive change in dementia care\, integrating personalised planning\, PBS principles and best practice standards. \nFind out more and apply here \nNote – This is an application\, not a booking.
