Frailty – Identification and Management for Managers – Cohort 5

Managers will learn how to communicate frailty more effectively with health care professionals from the knowledge gained by staff. This will allow best practice to help prevent deterioration in individuals and be able to action plan effectively to support individuals.

By the end of the course Management will be able to:
• To be able to communicate levels of frailty more effectively to health professionals and staff
• To prevent further deterioration in individuals by embedding the correct interventions
• Validated tool for identifying potential frailty and level of frailty
• To develop an action plan to be able to embed intervention effectively

Learners will be expected to be engaged in learning, participate in all activities including group discussions, share their own experiences of working in the care sector, whilst respecting confidentiality, and to share best practice examples.

A Onefile (e-portfolio) profile will be created on the learners behalf to support them in the completion of their course
