Course description
This course is aimed at identifying the varying levels of frailty within adult social care. Learners will have knowledge of the assessment tools needed to determine risk of frailty, have an awareness of what intervention methods are going to help reduce the risk of falls.
Learning outcomes
By the end of the course learners will be able to:
• What is frailty and how it relates to an ageing population
• What frailty looks like, what it’s made up of, and what it means to the individual
• Validated tools for identifying potential frailty and levels of frailty
• Practical experience of applying frailty assessment tools
Requirements and expectations
Learners will be expected to be engaged in learning, participate in all activities including group discussions, share their own experiences of working in the care sector, whilst respecting confidentiality, and to share best practice examples
A Onefile (e-portfolio) profile will be created on the learners behalf to support them in the completion of their course
